Description

Covers the $95 Suffolk County permit fee & the $400 event fee. These fees are not a donation or charitable contribution. Purchasing a spot in the event alone is not permission to attend and serve the event. In order to attend, you must complete the following no later than October 1st:

1. Complete and submit the HCC Registration form
2. Complete and submit the Suffolk County Temporary Food Service Vendor Application and Permit paperwork here
3. Provide proof of general liability and food service insurance for the event.
4. Complete and submit a W-9 form

Failure to complete this supplemental paperwork will lead to non-admittance to the event.
